The Maryland Department of Health and Mental Hygiene (DHMH) requires that every student submit an immunization certification form by the beginning of the school year if an updated form is not already on file; see the DHMH website (dhmh.maryland.gov) for details.

Please mail completed forms to Dorothy Reilly in the Admissions Office (301-365-0227 x176 or dreilly@heights.edu) or submit them via the admissions & enrollment portal (heights.schooladminonline.com/portal).
